Labeled 30-784A on the back. I've also got all the other parts ill list later. One fully functional reformed 12 dual voice coil woofer out of a Pioneer DSS-9 speaker. I've used it like this for a year with no issues.

Also reached thanks. Theres a small crack near the edge that's been epoxied on both sides to keep it from spreading see pic.
